110-300-0210(3) Licensor reviewed five additional child files at a health and safety recheck and observed three children did not have a complete CIS form. SW, AW, and HW did not have a signed and dated statement detailing when the child’s immunizations will be brought up to date and stating they understand their child will be excluded from care if the immunizations are not completed within 30 days of the specified due date.
